Does anyone have any restaurant or activity recommendations for a couples trip in Thailand? Specifically in either Changmai, Bangkok, Phuket, Phi Phi Islands or Krabi would be great. Thanks very much in advance!

Chang Mai elephant sanctuary is a must! You get to spend a whole day with the elephants and feed them etc! Other highlights include temple tours, the night bazaar market for shopping and tasty street food, and Woo - a lovely riverside restaurant which serves the best spring rolls.
I would also recommend venturing a bit further north to Pai which is beautiful.
For something fancy, get drinks at the newly opened Standard Hotel in Bangkok and book treatments at Rakxa spa - very reasonable but feels v special!
